Contact Form Setup

That layout form will only work with flodesk.  If you use flodesk you would create a form like that in your account, with the 3 inputs and then submit button.  Add your code replacing what is in that block and it will appear like that.  If you use another provider like mailchimp, or any of the others, it will not do the button sideways.  You would just replace the code with your form code.  I do believe that we can get mailchimp to have a side button like that if you email login details.
