Footer Like the Demo Issue

The demo has a “Stay In the Know” form in the footer, but the form does’t show up on the services page at jillconyers.com.

What do I need to do to add the form as it appears on the demo?

And, on the same page how do I delete the space below “Services” and above the first block? I tried the directions in the video about fixing the gap. Thank you.
